TROOP 461

| |
Paperwork:
To join, you will need the Youth Application and medical
forms.
 |
Youth Application.
 |
Just ask for a New Youth Application at a Troop
meeting. Scout shops have them too.
|
 |
The $10 registration fee, plus a $1.00 insurance fee, is $11.00 per
year.
 |
The $10
registration fee is scheduled to increase $15 on January 1st 2010.
The last fee increase, which was from $7 to $10, was in 2003.
|
|
 |
If transferring from another troop or cub
scout pack, the fee is only $1.00. Use the same New Youth
Application above and check the 'Transfer application' box near the
top.
|
 |
A Boy's Life Magazine subscription, at $12
per year, is optional.
|
 |
Adult Applications and Merit Badge
Counselor Applications are available as well.

 |
New Annual Health and Medical Record.
 |
Part A and Part C: All registered
unit members are required to provide both part A
(health history) and part C (parental consent) before attending
outings such as an overnight camping trip.
|
 |
Part B
(the physical examination) should be completed if you are
participating in an event that exceeds 72 consecutive hours, such as
jamborees, summer camps, and Wood Badge training courses.
 |
Tip: If you get a physical done
for school or sports any time during the year, take along
Part B and ask the
Doctor [i.e. licensed health-care
provider—physician (MD, DO), nurse practitioner, or physician’s
assistant] to sign it for you (hopefully free) and you're
all set for summer camp next June. Reason - Any physicals we
have on file dated prior to June 1st 2009 will not be valid for
Boxwell Summer Camp 2010.
|
 |
Tip #2: Physician's Medical
Walk-in Clinic (in the old Highland Rim Medical Center at 2893
Huntsville Hwy. in Park City, 297-5028) offers a $35 physical. They do not take appointments, you just walk in. It's
never been busy, and you are done in just minutes. Thanks to Dee &
Teddy for this handy piece of information.
|
 |
Tip #3: For those procrastinators
who will wait until the last minute before Boxwell Summer Camp to
get the annual physical done - there is a light at the end of the
tunnel for you. Try to get part B dated on June 1st or
after. That physical will be effective until June 30th
of the following year. Which means, you will not need to do
the physical again at all for Boxwell the following year.
One physical = two years of Boxwell Summer Camp. Who says the early bird always catches the worm?
